📂 Video Evidence – Dental Implant Complication: Sinus Perforation, Oral–Sinus Opening for Over Three Months, Infection and Subsequent Sinus Inflammation

Video Evidence – Persistent Oral–Sinus Opening Following Implant Complication During a dental procedure, five implants were placed. According to my account and the X-ray taken immediately after suturing, one of the implants had been incorrectly positioned and had entered the sinus region at an abnormal angle. The dentist attempted to remove the implant. During this attempt, the implant was pushed progressively deeper into the sinus area rather than being removed through the original opening. A subsequent X-ray showed the implant located much higher in the sinus/facial area, close to the region beneath my eye. The dentist then closed the surgical site with sutures and informed me that the implant would eventually come out or disappear from that location on its own. On later X-rays, the implant was no longer visible. Following this incident, however, an open passage between my mouth and the sinus region remained for more than two months. The opening failed to close despite repeated suturing. Air continuously passed through the opening, demonstrating that the oral cavity and sinus area were still communicating. Video 1 – Cigarette/Airflow Demonstration This video was recorded and sent directly to the treating dentist. In the video, I demonstrate the persistent opening by attempting to draw on a cigarette. Because air escapes through the abnormal opening, normal suction cannot be created. The video therefore documents, in real time, the continuing passage of air between my mouth and the sinus region after the implant complication. Video 2 – Water/Suction Demonstration This video was also recorded and sent directly to the treating dentist. I demonstrate the same defect by attempting to draw/drink water. Normal suction cannot be maintained because air passes through the opening. This provides additional visual evidence that the passage between the oral cavity and sinus region remained open for an extended period after the procedure. The significance of these videos is that they document the physical condition that existed after the implant incident: a persistent open communication between the mouth and sinus region lasting for more than two months. Such an opening potentially allows oral fluids, food particles and microorganisms to enter the sinus area and therefore presents a risk of infection and sinus complications. These videos are preserved as contemporaneous evidence of the postoperative condition and were originally sent to the treating dentist to demonstrate that the opening had not healed.
